Historical Marker Application Form

This application form is used for obtaining state historical markers and it must be filled out completely. The applicant is responsible for submitting the required information and documentation, for funding the historical marker, and for future maintenance for the historical marker. Please note, submission of an application does not guarantee that a marker will be awarded.
